Seaside Home Health Care Social Workers provide a variety of care services that allow seniors to remain in their homes. These services are meant to enhance the quality of life for the senior. Our Social Workers help clients, and their families meet the challenges of aging with dignity, pride, care, and compassion.
The Social Worker’s responsibilities include performing client visits and monitoring their conditions, as well as providing education and counseling services to clients and their families. Our Social Workers are expected to address all the client’s concerns and make appropriate adjustments to the client’s in-home plan of care.
To be successful as a Seaside Home Health Care Social Worker, you should be a nurturing individual who can handle a wide range of tasks while building a relationship with the client and their family. It is also important for our Social Workers to be able to separate the client from his/her situation, limitations, or disease. Outstanding candidates possess the mental strength and resilience to care for clients and are also able to provide emotional support to family members.
PRIMARY RESPONSIBILITIES
- Reflect the core values of Seaside Home Health Care, LLC
- Contribute to a positive living environment to enhance the client’s quality of life, including, listening to the concerns of family members, and answering their questions
- Determine nature of client’s situation by interviewing client, assessing medical, psychological, emotional, and social information, and making on-site visits
- Document daily activities and report any significant changes in a client’s needs or living conditions to the office
- Establishes course of action by exploring options and setting goals with the client
- Obtains assistance for client by referring him/her to community resources, arranging for appointments and establishing rapport with other agencies
- Monitors planned actions by periodic follow-up calls/visits
- Follow all guidelines set forth in Seaside Home Health Care Policies, Scopes of Service, and Client’s Plan of Care
- Counseling clients and family members through the diagnosis, treatment, and emotional implications of their conditions.
- Educating caregivers and family on the aftercare or ongoing care of the client
- Providing suggestions for improved healthcare to physicians and family members of the client
- Monitoring patient recovery and compiling reports for the physician
- Keeping abreast of developments in healthcare and attending workshops and lectures as required
- Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for clients
- Maintain regular communication with supervisor and office staff
- Utilize operations software portal to stay up to date on schedule and client information
- Maintains client confidence and protects operations by keeping information confidential
- Respond to office communication within two hours, or when you visit ends
SECONDARY RESPONSIBILITIES
- Participate in in-service and continuing education training and mandatory employee meetings
- Manage availability and communicate changes in a timely manner
- Participate in regular Satisfaction Surveys
- Participate in ongoing Quality Improvement programs
- Document client information electronically after every visit
- Perform other reasonable duties as assigned
ESSENTIAL JOB REQUIREMENTS
- Master’s Degree in social work accredited by the Council of Social Work Education
- Licensed clinical social worker preferred
- 2-5 years related experience
- State-specific licensure required
- State-specific driver’s license required
- Excellent observational and problem-solving skills
- The ability to follow instructions but also to act independently as necessary
- Excellent report-writing skills
- Active listening skills and empathy
- Physical and mental strength
