Seaside Home Health Care’s Licensed Practical Nurses provide a variety of care services that allow seniors to remain in their homes. These services are meant to enhance the quality of life for seniors. Our nurses help clients, and their families meet the challenges of aging with dignity, pride, care and compassion.

The Nurse’s responsibilities include performing client visits, monitoring/assessing their condition, and updating changes to Physicians. They should be able to address all the client’s concerns and make appropriate adjustments to the client’s in-home plan of care by providing professional services and care to seniors through a human-centered interdisciplinary approach.

To be successful as a Seaside Home Health Care Nurse, you should be a nurturing individual who can handle a wide range of tasks while building a relationship with the client. It is also important for our nurses to be able to separate the client from his/her situation, limitations, or disease process. Ideal candidates possess the mental strength and resilience to care for clients and are also able to provide emotional support to family members.

The Licensed Practical Nurse reports to the Nurse Clinical Manager. Your employer is Bidwell Care Holdings, LLC dba Seaside Home Health Care.

PRIMARY RESPONSIBILITIES

  • Reflect the core values of Bidwell Care Holdings, LLC
  • Contribute to a positive living environment to enhance a client’s quality of life
  • Report any significant changes in a client’s needs and medical concerns to Clinical Nurse Manager and to client’s physician.
  • Update care plan as needed
  • Maintain regular communication with clinical nurse manager and client’s physician as appropriate
  • Utilize software to stay up to date on schedule and client information
  • Follow all guidelines set forth in Seaside Home Health Care Policies, Scopes of Service, and Client’s Plan of Care
  • Traveling to client’s homes and managing their care plans according to physicians’ instructions.
  • Completing appropriate nursing documentation to include any medication changes, changes in condition and reporting to Clinical Nurse Manager and physician as appropriate
  • Listening to the concerns of family members and answering their questions
  • Educating client, their family/caregiver on identified goals and interventions to promote positive outcomes in plan of care to prevent ER/hospital admissions or readmissions
  • Maintain communication with clinical team, Physician

SECONDARY RESPONSIBILITIES

  • Participate in in-service and continuing education training and mandatory employee meetings
  • Participate in regular Satisfaction Surveys
  • Maintain documentation completion in a timely manner
  • Perform other reasonable duties as assigned

ESSENTIAL JOB REQUIREMENTS

  • Graduate of an accredited school of nursing
  • A valid LPN license in the state of Florida
  • Excellent observational and problem-solving skills
  • Excellent critical thinking, and interpersonal communication skills
  • Ability to make ethical decisions
  • Ability to follow instructions but also act independently
  • The ability to know when to reach out to other members of the clinical team for advice or assistance as appropriate
  • Excellent active listening skills and empathy
  • A valid driver’s license and reliable transportation
