Seaside Home Health Care Physical Therapists provide a variety of care services that allow seniors to remain in their homes. These services are meant to enhance the quality of life for the seniors. Our Physical Therapists help clients, and their families meet the challenges of aging with dignity, care, and compassion.
The Physical Therapists’ responsibilities include performing client visits and monitoring their conditions, as well as providing education and counseling services to clients and their families.
A successful Seaside Home Health Care Physical Therapist should be a nurturing individual who can handle a wide range of tasks while building a relationship with the client and their family. This individual would act as our client’s partner and advocate on their road to recovery, from diagnosis to the completion of rehabilitation. Client quality of life, range of motion, and overall safety should be the sole focus of any candidate applying for this position.
The Physical Therapist reports to the Alternate Administrator.
PRIMARY RESPONSIBILITIES
- Reflect the core values of Seaside Home Health Care, LLC
- Contribute to a positive living environment to enhance the client’s quality of life, including, listening to the concerns of family members, and answering their questions
- Document daily activities and report any significant changes in a client’s needs or living conditions to the office
- Meet with clients and identify needs and goals
- Devise detailed treatment plans to achieve needs and goals
- Motivate and encourage patients throughout the treatment process
- Devise detailed exercise techniques, stretches, and self-care programs for clients to complete between sessions
- Evaluate effects of therapy and keep detailed records of client appointments and progress
- Communicate with clients’ physicians regularly to ensure consistent care
- Reduce clients’ need for medications and surgeries by improving range of motion, strength, flexibility, coordination, and balance
- Comply with federal and state physical therapy regulations
- Maintain regular communication with supervisor and office staff
- Utilize Operational Software Portal to stay up to date on schedule and client information
- Maintains client confidence and protects operations by keeping information confidential
- Respond to office communication within two hours, or when the visit ends
SECONDARY RESPONSIBILITIES
- Participate in In-Service and continuing education training and mandatory employee meetings
- Manage availability and communicate changes in a timely manner
- Participate in regular Satisfaction Surveys
- Participate in ongoing Quality Improvement programs
- Document client information electronically after every visit
- Perform other reasonable duties as assigned
ESSENTIAL JOB REQUIREMENTS
- Must hold a Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program and obtain a state license
- 2-5 years related experience
- State-specific licensure required
- State-specific driver’s license required
- Excellent observational and problem-solving skills
- The ability to follow instructions but also to act independently if necessary
- Excellent report-writing skills
- Active listening skills and empathy
- Physical and mental strength
